Now is the time to schedule your child’s Sport/School Physical. Call now to avoid the summer rush! Appointments will book up fast! All sports physicals must be completed in order to participate in sports. *Please note that pages 1-4 must be completed by parent prior to the appointment.*
When we see you for your well child visit, you can expect the following:
– A Physical Exam
– Evaluation of your developmental milestones
– Administration of important immunizations such as: Tdap, HPV and Meningitis.
– Sports forms will be completed and signed
– Your child must be fully immunized in order to go to school!
– Please call soon to schedule your child’s physical, appointments fill up quickly!
